TransCelerate AI Pharmacovigilance: FDA & EMA Roadmap

Executive Summary
This report provides an in‐depth analysis of TransCelerate BioPharma’s guidance on implementing artificial intelligence (AI) in pharmacovigilance (PV), with a particular focus on the regulatory environments of the U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA). We begin by reviewing the role of TransCelerate as a pharmaceutical industry consortium and its initiatives to promote AI and automation in drug safety. We then survey the state of AI in PV, citing industry surveys and pilot studies showing that by 2021 “a large number of companies were either planning to use or had already started using intelligent automation tools in their PV processes” ([1]), and noting that workflow automation targets the “strongest cost driver” in PV operations ([2]). Throughout, transparency and collaboration across stakeholders (sponsors, regulators, technology vendors) is emphasized.
The report is supported by extensive literature: industry case studies and surveys ([1]) ([9]), regulatory documents ([7]) ([5]), and TransCelerate publications ([10]) ([11]). Two tables illustrate (1) how TransCelerate’s AI/PV guideline topics map onto FDA and EMA requirements, and (2) a step‐by‐step roadmap with associated regulatory considerations. We conclude that successful implementation of AI in PV, underpinned by TransCelerate’s guidance, can significantly enhance drug safety monitoring, but requires meticulous alignment with evolving FDA and EMA regulations to ensure patient safety and data integrity ([12]) ([7]).
Introduction and Background
Pharmacovigilance (PV) is the science and process of monitoring the safety of medicines and taking action to reduce risks and increase benefits. PV systems traditionally rely on human review of Individual Case Safety Reports (ICSRs) and statistical signal detection methods. However, the volume, variety, and velocity of safety data have exploded with global drug overviews and digital reporting. This has made routine PV tasks (case intake, coding, literature screening, signal detection) increasingly burdensome and error-prone ([13]) ([2]). Operators note that ICSR processing is “the strongest cost driver” of PV budgets, given the manual labor involved ([2]). As a result, the pharmaceutical industry is turning to intelligent automation and artificial intelligence to improve efficiency and consistency in PV.
In fact, surveys conducted by industry groups (such as TransCelerate BioPharma and PVnet) have found that by 2021 most (often large) companies were either piloting or already using AI and robotics in PV ([13]). The use cases include automated text mining (NLP) of case narratives, robotic data entry for ICSRs, and AI-assisted signal triage. One pharmaceutical pilot showed that an AI/RPA system could automatically extract patient age, gender, drug dose, and adverse event information from source documents with high accuracy ([14]). Overall, industry consensus is that “it is feasible to apply AI to automate safety case processing” under controlled conditions ([9]), which can free skilled professionals to concentrate on medical evaluation and new safety science ([15]) ([2]).
The regulatory landscape governing PV is stringent.In the U.S., sponsors must meet FDA regulations (e.g. 21 CFR Parts 314, 600) for timely reporting of safety cases and undertake proper computerized system validation (CSV) per 21 CFR Part 11 for electronic records. In Europe, companies must comply with Good Pharmacovigilance Practices (GVP) as outlined in the EU’s EudraLex, volume 9A, and with Annex XI of the EU GMP for computerized systems. The use of any significant automated or AI-enabled system in PV implicates these rules: as one TransCelerate analysis noted, “when such technology solutions are implemented to assist in processing AE cases, regulations require pharmaceutical companies to validate this software.” ([16]).
TransCelerate BioPharma, a consortium of leading drug companies, has made AI in PV a strategic priority. Its Intelligent Automation Opportunities in Pharmacovigilance initiative (completed in 2025) has identified use‐cases for AI/ML and produced guidance on technology evaluation and validation ([17]) ([11]). TransCelerate’s mission is to facilitate harmonization among health authorities and share industry best practices ([10]). In the AI/PV domain, TransCelerate’s published works (e.g. surveys, case studies) inform how to combine new tools with regulatory requirements. For example, their ICSR automation pilot study provided “strong evidence” that AI can reduce workload and improve consistency ([18]), and their position papers draw analogies to Good Manufacturing Practice (GMP) approaches for validating automated systems ([19]).
This report examines TransCelerate’s AI in PV guidelines (actual or conceptual) and lays out an implementation roadmap for pharmaceutical companies to adopt these in compliance with both FDA and EMA regs. We cover historical context (the slow uptake of AI compared to other sectors ([12])), current PV and regulatory perspectives (citing recent FDA/EMA statements on AI in medicines ([7]) ([5])), and practical steps for bridging TransCelerate’s guidance to official requirements. Case studies (such as the Pfizer ICSR pilot) and technical surveys are used to ground the discussion in real-world data ([9]) ([1]). Throughout, we emphasize evidence-based analysis, multiple viewpoints (industry, regulators, patients), and fully reference all claims.
TransCelerate’s AI/Pharmacovigilance Guidelines and Industry Perspectives
TransCelerate does not (publicly) call them “AI guidelines” specifically, but its PV initiatives effectively produce guidance on using AI and automation responsibly. The core TransCelerate philosophy is collaborative harmonization: identifying ambiguous regulations and “proposing a reasonable interpretation” or best practice that meets the intent of PV laws ([20]) ([21]). The consortium explicitly aims to “facilitate sharing of safety information” and build trust with regulators and patients ([10]). In practice, its workgroups have:

Released case study summary themes for AI validation. Recognizing that novel AI tools don’t fit neatly into 1990s‐era validation guidance, TransCelerate’s position paper recommends a risk‐based, lifecycle approach. It borrows the GAMP 5 strategy from GMP (focus on intended use, risk ranking) and incorporates FDA’s concept of Good Machine Learning Practice ([24]). Convened pilot studies. Perhaps most consequentially, TransCelerate sponsored a landmark 2018 pilot where three commercial AI/RPA vendors processed real ICSR cases from multiple companies ([8]). The trial measures (multi-cycle) demonstrated that AI engines could reliably extract key case fields from source documents ([14]), even without costly manual annotation. “The results from the pilot demonstrated that it is feasible to apply AI to automate safety case processing,” the authors reported ([9]). This pilot has served as proof‐of‐concept: it “provided strong evidence that these technologies can significantly reduce workload, improve consistency, and support faster handling of safety data” ([18]). The success of this study has been widely cited (even by regulators) as industry evidence that AI tools can meet high standards for case processing.
Taken together, TransCelerate’s outputs constitute a de facto set of AI/PV guidelines. Although not binding regulation, they articulate principles such as: maintain data integrity and traceability, employ risk‐based validation, ensure clinical oversight, and engage regulators proactively. Many of these principles echo existing ICH and FDA standards (e.g. risk-based CSV per ICH Q9) but apply them to modern AI tools. For example, if FDA published guidance calls for a Predetermined Change Control Plan for ML medical devices ([25]), TransCelerate would advise similarly that any AI system includes a protocol for future training updates. Likewise, if EMA’s draft reflection paper highlights lifecycle responsibilities for AI models, TransCelerate’s materials would emphasize that sponsors document concepts such as the data provenance and the continued validity of the model.

The above table illustrates that TransCelerate’s guidelines – which emphasize data quality, risk-based validation, human oversight, and compliance – are largely consistent with both FDA and EMA mandates. In many cases TransCelerate is essentially translating regulatory intent into PV-specific best practices (for example, the requirement to retain ICSR data integrity ([9]) is handled through ALCOA+ data governance). The guiding principles being jointly developed by FDA and EMA (ten good-practice principles covering AI across development and safety) overlap strongly with TransCelerate’s focus on accountability, traceability, and human involvement ([7]).
Regulatory Landscape: FDA Perspective
From the FDA’s standpoint, the critical questions are how to incorporate AI/ML tools into PV systems without compromising safety or compliance. Historically, the FDA’s regulations have not explicitly addressed AI, but the agency is rapidly developing frameworks. Crucial existing requirements include (a) 21 CFR Part 11, which governs electronic records and signatures, mandating validated systems and audit trails for any data used in regulatory submissions; and (b) 21 CFR 314/600/606 on safety reporting (e.g. MedWatch, or 112-minute expedited reports). Under Part 11, any software that creates or modifies electronic records (including an AI algorithm classifying cases) must be validated and under change control. As one TransCelerate paper bluntly puts it, “regulations require pharmaceutical companies to validate [any] software” used in case processing ([16]). Thus even state-of-the-art AI systems must be integrated into a GxP-compliant environment.
FDA Initiatives on AI. The FDA has signaled strong interest in AI/ML. Notably, in January 2025 the International Medical Device Regulators Forum (IMDRF) – including FDA – released ten Good Machine Learning Practice (GMLP) guiding principles for AI in medical devices ([3]). These are not PV-specific, but they apply equally to any clinical AI. They emphasize a total product lifecycle approach (including post‐market changes) and rigorous validation. The FDA has also teamed with Health Canada and others to publish guidance on AI-enabled medical devices (e.g. transparency requirements, change-control plans ([25])).
For pharmacovigilance, FDA programs are emerging. In June 2024 the FDA CERSI (Center for Excellence in Regulatory Science) announced a research project on “Quality assurance for AI algorithms in PV” ([4]). This project, led by FDA scientists and academic collaborators, aims to survey literature on how to evaluate AI in safety work. Its objectives include identifying methods to measure AI performance and ensure robust QA processes ([4]) ([30]). The very existence of this project shows the FDA’s commitment to understanding AI validation – and suggests that industry best practices (like TransCelerate recommendations) will be aligned with formal QA methods in the near future.
FDA Enforcement and Approvals. Although there is no FDA-approved “AI-based PV system” label, FDA has already approved many AI-embedded medical devices (e.g. diagnostic software) under 510(k) or De Novo pathways. These approvals highlight FDA’s concerns about bias, cyber security, and patient safety. In PV specifically, FDA experts (such as Dr. Gerald Dal Pan) have publicly discussed how AI can be used for signal detection—but only under controlled conditions ([7]) ([26]). The recent FDA statements (with EMA) call for transparency, reproducibility, and human oversight in AI applications ([7]). In practical terms, this means a sponsor must be able to explain how an AI tool came to a certain safety recommendation or warned of a signal, and must intervene when the model is uncertain.
PV Database Systems. From a PV operations viewpoint, companies already maintain validated safety databases (e.g. Argus, ArisGlobal) subject to FDA rules. Embedding AI into these systems generally takes two forms: upgrading the database with AI-capable modules, or interfacing AI engines externally. In either case, any change triggers re-validation under FDA’s Computer Software Assurance (CSA) approach. FDA’s own guidance on software validation (in the context of AI, via GAMP 5 analogues) calls for risk-based approaches ([28]) ([16]). These documents recommend documenting intended use, potential failure modes, and verifying outputs with testing on both training and unseen data.
In summary, FDA requires that any TransCelerate AI/PV solution be implemented within a compliant quality system. This means: validated design (with user requirements implemented), audit trails on algorithms, controlled access, and documented qualification steps – exactly the kind of processes outlined in TransCelerate’s position paper ([28]). Additionally, the sponsor must ensure that automated decisions do not violate FDA reporting timelines or case completeness requirements. For example, if AI triages an ICSR as “likely non-adverse,” the company still must review or search for any missed reportable events. Throughout, the FDA’s guiding concern is patient safety: as one industry commentary notes, all AI-based PV systems must preserve the FDA’s mandate of timely, accurate adverse event reporting ([31]) ([7]).
Regulatory Landscape: EMA Perspective
The EMA and national EU agencies share FDA’s interest in AI but operate within Europe’s own legal framework. EU pharmacovigilance is governed by Directive 2001/83/EC and subsequent regulations, which require Marketing Authorisation Holders to monitor safety and report ICSRs to EudraVigilance on strict timetables. EMA’s GVP guides (Modules V–VIII) detail how to detect signals and report them, but until recently they did not explicitly address AI or advanced analytics. However, the European regulatory network has rapidly instituted AI in its strategic planning.
EMA’s AI Strategy. In September 2023, the EMA published a draft reflection paper on AI in the medicinal product lifecycle ([6]), signaling that all stakeholders (sponsors, regulators, academics) should prepare for AI’s impact. The reflection paper (finalized in 2024) outlines general principles rather than concrete rules, emphasizing data traceability, risk management, and cross-stakeholder collaboration. It followed a November 2023 multi-stakeholder workshop and public consultation, showing EMA’s intention to harmonize viewpoints.
More tangibly, the EMA’s Network Data Steering Group (NDSG) – a committee of EU regulators – launched an AI workstream in its 2025–2028 plan ([32]) ([5]). That plan explicitly includes “guidance, policy and product support – delivering guidance on the use of AI throughout the medicine lifecycle” and “tools and technology – frameworks for AI tools” ([5]). In plain terms, this means EMA will develop official guidance (e.g. updated GVP modules) and possibly templates or standards for AI in drug development and safety. EMA also coordinates with international bodies: both EMA and FDA are members of the International Coalition of Medicines Regulatory Authorities (ICMRA) working group on innovation, which now explicitly includes AI.
Joint FDA–EMA Initiatives. Significantly, in January 2026 the FDA and EMA announced a bilateral collaboration on AI in medicines ([7]). A cornerstone output was the publication of joint guiding principles for good AI practice in drug development. These ten principles, adapted from FDA’s GMLP initiative, apply to manufacturers and MAHs of medicines (not just devices) and cover all phases from early research to post-marketing surveillance. They stress human-centric values and patient-focused outcomes. Although not legally binding, these principles effectively serve as global best-practices. They explicitly note that translates to PV by stating that “Principles on evidence generation and monitoring across all phases … including safety monitoring” are addressed ([33]) ([7]). Both agencies emphasize that AI systems should be transparent, reproducible and under competent human oversight ([7]) – in line with TransCelerate’s recommendations.
EU Legal Environment. On the legal side, the forthcoming EU AI Act (expected 2024) will classify AI tools used in drug safety as “high-risk” systems, subject to strict obligations on data governance, documentation, transparency, and human oversight. The AI Act requires providers to demonstrate compliance (e.g. risk assessments, records) before marketing high-risk AI. While the Act is still being finalized, companies must already start aligning with its principles. In parallel, existing EU data-privacy laws (GDPR) and EudraLex volume VII (clinical research)/IX (marketing) demand data security and consent for any health data used in training models.
EMA’s Practical Requirements. Even without new laws, EMA expects that adoption of AI follows the same underlying GxP ethos. For example, EMA’s Annex 11 (Computerized Systems) and EudraLex Volume 4 on GMP describe the need for system qualification and risk analysis. The EMA reflection paper hints that AI models influencing PV must undergo comparable checks as any computerized batch release decision. EMA’s EV (VigiBase) database already uses probabilistic algorithms (vigiMatch, vigiRank) to detect duplicate reports and signals, demonstrating that regulators themselves are applying AI ([34]). In fact, the Uppsala Monitoring Centre (operating VigiBase) successfully employed machine learning to scan massive datasets of EHR notes for ADRs. These cases highlight that regulators not only permit, but increasingly rely on AI, provided it is validated and human-reviewed.

Thus, the EMA environment can be summarized as “encouraging innovation under careful control.” The EMA’s own statements put it succinctly: EU regulators aim to “maximise the benefits from AI while ensuring that uncertainty is adequately explored, and risks are mitigated” ([35]). Companies should note that the EMA is actively developing concrete deliverables (e.g. final guidelines) under its AI workstream, so continued regulatory dialogue is crucial.
Implementation Roadmap
Translating TransCelerate’s AI/PV guidelines into practice under FDA/EMA regimes requires a structured roadmap. This roadmap must blend technical adoption with regulatory compliance. Based on industry best practices and regulatory expectations, we propose the following phased approach:

Current State Assessment (Gap Analysis). Identify PV Processes and Data Requirements. Map the end-to-end ICSR workflow (intake, validation, coding, narrative writing, signal detection) and identify which tasks bring value from AI or RPA. (TransCelerate’s ICSR survey can guide this step ([22]).) Catalog existing IT systems, data standards (SDTM, MedDRA), and quality controls. Assess IT infrastructure readiness (e.g. availability of annotated datasets, EHR links). Regulatory focus: Determine which FDA/EMA requirements currently apply to these processes (e.g. Part 11 controls on databases, GVP modules for reporting). Identify potential gaps in compliance if automation is introduced(Table 1 above captures key areas). Use this gap analysis to prioritize AI initiatives (e.g. start with structured data tasks like duplicate detection or seriousness coding before text-heavy NLP).

Governance and Policy Development. Define AI Governance Framework. Establish an internal AI governance team comprising PV experts, quality/compliance, IT, and legal. Develop policies outlining roles (e.g., data scientists, PV reviewers, validators), responsibilities, and oversight procedures. Specify criteria for vendor selection, data handling, and change control. Document how AI outputs will be reviewed by qualified personnel (e.g. a PV physician or QPPV) before final decisions.
