The pharmaceutical industry’s reliance on computer systems — from laboratory instruments ([10], HPLC) to manufacturing execution systems (MES), enterprise resource planning (ERP), and electronic documentation — has grown significantly over the past decades. As a result, regulators expect firms to validate these systems to prevent errors and ensure compliance. U.S. FDA regulations (e.g. 21 CFR Part 11 for electronic records and 21 CFR 211.68 for equipment control) explicitly require controls and validation to assure data accuracy under CGMP ([11]) ([12]). Similarly, the EU’s GMP Annex 11 (Computerized Systems) mandates validation and documentation of computer systems. International guidelines like ICH Q9 (Quality Risk Management) and Q10/Q12 reinforce bringing risk management into quality systems.

CSV Benefits and Drivers. A recent review notes that comprehensive CSV yields multiple benefits: it improves product quality, reduces validation time and cost, and increases confidence in GMP compliance (e.g. with Part 11) ([13]). Evolution of Good Automated Manufacturing Practice (GAMP). To guide industry in pragmatic CSV approaches, the ISPE (International Society for Pharmaceutical Engineering) introduced GAMP in 1991 (UK) to complement evolving FDA GMP expectations ([16]) ([1]). GAMP has always offered risk-based, life-cycle methodologies and common terminology for validating GxP systems ([9]) ([2]). Its core principle is that quality must be built into systems at each stage (not merely tested in at the end) ([17]). GAMP publications have evolved: GAMP 5 (first edition, 2008) provided a comprehensive framework emphasizing lifecycle and categorization, and it remains the de facto international guidance ([18]) ([1]). In July 2022, ISPE released GAMP® 5 Second Edition, updating the guidance to address modern technologies and practices ([1]).This report centers on the 2026 landscape of CSV implementation under GAMP 5 Second Edition, highlighting the interplay of legacy principles and new trends (e.g., cloud, AI). We will examine the regulatory background, GAMP’s philosophy, practical implementation strategies (the “Implementation Workbook”), and the future outlook of CSV in pharma.

Regulatory Landscape for CSV in Pharma

US FDA Requirements. In the United States, 21 CFR 211.68 outlines that any automatic, mechanical, or electronic equipment (including computers) used in drug manufacturing must be routinely calibrated and inspected under a written program ([19]). Crucially, 211.68(b) requires “appropriate controls” over computer systems to ensure only authorized changes to records and that input/output data are verified for accuracy, with verification frequency based on system complexity ([11]). While Part 211 does not explicitly say “validate”, FDA’s enforcement (483s/warning letters) has clarified that risk-based validation is expected as part of CGMP. Moreover, 21 CFR Part 11 (Electronic Records/Electronic Signatures) (1997) sets foundational requirements for electronic documents in clinical and manufacturing environments ([12]). Recent FDA guidance emphasizes a risk-based approach to Part 11 compliance, focusing on patient safety and data integrity in the digital age. For example, an FDA final guidance (Oct 2024) on Part 11 in clinical trials instructs stakeholders to use appropriate controls and technology to maintain record traceability ([20]). In practice, U.S. regulators and inspectors routinely check system validation, audit trails, and data governance as part of CGMP audits.

EMEA and PIC/S Requirements. The European Union’s EudraLex Volume 4 Annex 11 (Computerised Systems) similarly requires validation throughout the system life-cycle ([21]). The existing Annex 11 (2011 version) mirrors 21 CFR 211.68’s intent: emphasizing validation, audit trails, data security, and documented procedures for computer systems ([11]) ([21]). Starting around 2019, however, the EU recognized that Annex 11 needed updating for new technology trends (cloud, AI, digital records). In late 2022 the EMA released a “Concept Paper” proposing widespread revisions, including requirements for data integrity (beyond at-rest), data in motion, digital transformation, and AI/ML usage ([21]) ([22]). Pubic consultation closed in October 2025, and a draft new Annex 11 was expected by March 2025 with final adoption slated for 2026 ([7]). Concurring with the EU, the PIC/S (Pharmaceutical Inspection Co-operation Scheme) has adopted identical Annex 11 content (PIC/S PI 011-3) and will likewise mirror the revisions ([23]).

Data Integrity Guidance. A key regulatory theme of recent years is data integrity (ALCOA+). U.S. FDA and EMA have issued strict guidance on assuring data integrity in GMP systems. For instance, a 2021 FDA guidance and related industry analyses note that between 2017-2022, over 160 FDA warning letters cited GMP data-integrity deficiencies, reinforcing the need for audit trails, timestamping, and full data accountability ([24]). The ALCOA+ acronym (Attributable, Legible, Contemporaneous, Original, Accurate, plus Complete, Consistent, Enduring, Available) is embedded in policy as an inspection benchmark ([24]). FDA explicitly notes that CSV must consider ALCOA+/21 CFR Part 11 elements equivalently for electronic systems, and EU regulators likewise stress these principles in Annex 11 drafts and QRM guidelines. The result is that CSV in 2026 cannot ignore expansive data governance: processes must capture metadata (audit trails, versioning), QA must review these systematically, and electronic signatures must be tightly controlled.

Quality Risk Management (ICH Q9). Both GAMP and regulators explicitly require a risk-based approach, aligned with ICH Q9 principles. GAMP 5 and Annex 11 instruct firms to categorize systems and tailor validation effort based on risk (impact on product quality/patient safety). Risk Management tools (e.g. FMEA, risk ranking) are used to prioritize requirements and testing, as well as to periodically review system changes. Explicitly, GAMP embeds ICH Q9’s concepts into its framework ([25]) ([1]). Other Regulations. Other international standards intertwine with CSV: WHO GMP and PIC/S GMP guidelines reference the need for validated computerized processes. Countries like Japan (JP XI) align with EU Annex 11, and new regulations (e.g. FDA’s 2026 “Computer Software Assurance” draft for production/QMS software) encourage focusing validation on high-risk software aspects. Even data privacy laws (GDPR) now touch CSV: cloud-hosted EU data requires architectural controls and DPIAs as part of system design and validation plans. In sum, CSV today means not just proving software works, but maintaining robust controls over data security, privacy, and evolving tech.

Principles and Framework of GAMP 5

GAMP® Purpose and Scope. GAMP® is not a regulation itself, but a best-practice framework maintained by ISPE that helps companies interpret and implement regulatory expectations for computerized systems ([9]). The core philosophies are: keep the patient at center, use a risk-based, science-led approach, and focus on systems being “fit for intended use” ([9]). GAMP provides a common language for CSV projects – for instance, distinguishing user requirements, functional vs. design specifications, Good Documentation Practices (GDP), etc. It clarifies roles (user vs. supplier vs. quality) and emphasizes the entire system life-cycle from concept to retirement ([26]) ([1]). Importantly, GAMP guidance is pragmatic: it doesn’t dictate how to write code or tests but sets a flexible process framework and examples of good documentation to meet compliance efficiently ([27]) ([2]).

GAMP 5 – First Edition (2008). The 2008 GAMP 5 guide codified the regime into a six-step life cycle (in concept, project, development, test, release, operate/maintain) and a categorization model for systems. The system categories (Table 1) range from simple infrastructure software (Category 1) to complex custom applications (Category 5). The validation effort is scaled: Category 1 and standard non-configured off-the-shelf software require minimal validation (just installation checks), whereas higher category systems need full URS/FS/DS and rigorous testing ([2]) ([16]). GAMP 5 also established five key principles (e.g., leveraging suppliers, balancing risk, ensuring QA oversight) and a V-model for engineering – essentially integrating requirements, design, and testing phases with formal reviews. The guiding concept was that quality must be built in (cannot just be tested in at end), echoing GMP’s mandate that production consistency starts before manufacturing ([17]) ([2]).

Key GAMP 5 Principles (First Edition): Among GAMP’s guiding principles are the ideas that systems should be categorized by risk; validation approaches should be scaled appropriately; involvement of suppliers and documentation reuse is encouraged; and formal processes (change control, source code change management) are essential. GAMP stresses using risk-based quality management throughout – prioritizing system functions that could impact product or safety – rather than exhaustively testing low-risk areas. For example, functional testing may be limited to high-impact features, and spreadsheet macros might be tested selectively based on their complexity ([28]) ([1]).

GAMP 5 Second Edition (2022): The second edition retains the first edition’s core lifecycle model and risk-based ethos (still aligning with ICH Q9) ([1]) ([29]), but updates context for “today’s technologies and practices.” ISPE notes that GAMP 5 (2nd ed) continues to “protect patient safety, product quality, and data integrity” by encouraging systems that are “effective, reliable, and of high quality” ([30]) ([31]). The new edition maintains the principles and framework of GAMP 5 first edition but explicitly incorporates modern trends: greater supplier involvement, use of agile/iterative software models, and broader software tools/automation ([1]) ([32]). For example, it encourages regulated firms to leverage supplier documentation and testing evidence where possible (rather than redoing all testing in-house) ([1]). It also clarifies that GAMP’s V-model is not the only approach – incremental and agile methodologies are fully supported, as long as they include proper validation considerations ([1]). CSV Life-Cycle and Documentation

Under GAMP’s life-cycle model, CSV follows a structured sequence of phases, each producing specific deliverables and involving distinct activities. Figure 1 (below) illustrates a generic life-cycle aligned with GAMP 5, and Table 2 summarizes key documentation per phase.

In practice, not all life-cycle models look identical as drawn above, especially with agile/continuous processes, but the GAMP 5 Second Edition affirms the importance of defined phases and deliverables, suitably adapted to project methodology. For instance, lean/Agile CSV might combine OQ and PQ or iterate builds with incremental testing, but every identified requirement must be tested (risk-based), and records generated to show compliance ([1]) ([8]).

Project and CSV Master Plan. Early in the life-cycle, a CSV Master Plan (sometimes called the CSV Strategy) is created. This overarching plan describes the company’s approach to validation in general and for the specific system in particular. It outlines scope, roles (e.g. who is responsible in QA vs. IT vs. supplier), timelines, and the high-level risk assessment strategy. According to Cureus literature, “the life cycle of CSV starts from the planning stage...starting with a master plan and ending with periodic system reviews” ([8]). The Master Plan references applicable regulations (21 CFR, Annex 11 etc.) and GAMP guidelines, and may classify the system into risk categories.

Risk Management and Traceability. Once requirements are defined, GAMP 5 prescribes a traceability matrix linking User Requirements (URS) to specification elements and test cases. A risk assessment informs the level of scrutiny: high-impact requirements (e.g. those affecting product specs, safety alarms, or essential records) demand thorough testing and evidence; low-impact features may get minimal checks. Quality Risk Management (aligned to ICH Q9) is applied throughout – e.g., an FMEA-based workflow might be used to score risks of requirements, defects, and to prioritize testing effort.

Installation and Operational Qualification (IQ/OQ). Typical CSV delineates IQ (ensuring the system is installed per vendor specs) and OQ (ensuring the system functions as intended under normal modes). IQ may involve version checks, verifying network connections, and confirming necessary backend services are active. OQ tests key features using designed test scripts; for configurable systems, each major configuration setting is verified (e.g., a laboratory software’s calculation formula, a user role permission). All deviations during testing must be documented and resolved.

Performance Qualification and User Acceptance (PQ/UAT). After OQ, PQ (or UAT) demonstrates the system working end-to-end in an environment mirroring production, often using real or representative data. This may include, for example, processing an actual manufacturing batch in a test system to show results align with expectations. User acceptance testing, while often seen in software lifecycles, in CSV is still treated as formal qualification. Successful PQ allows QA to sign off for production readiness.

Post-Implementation & Change Control. Validation doesn’t stop at release. Once live, users must be trained, and the system enters the regulated environment. Any changes thereafter are stringently controlled: minor updates (patches, configuration tweaks) require impact analysis, testing, and possibly re-qualification under a change control procedure. GAMP 5 (2nd Ed) emphasizes that change management is “essential for maintaining the regulatory, legal, and operational integrity of the system” ([34]). Documentation is usually maintained electronically (some firms use test management or validation lifecycle management tools) to facilitate audits. Periodic reviews (often annually) reassess that the system remains in a validated state, especially if technology or GMP requirements have evolved.

Deliverables Summary. Key documents throughout CSV typically include the following (see Table 2):

  • CSV / Validation Master Plan – strategy, scope, roles, and risk approach.
  • User Requirements Specification (URS) – detailed requirements from stakeholders.
  • Functional Specification (FS) – optional document for COTS / configurable systems, describing system functionality (often from vendor).
  • Design Specification (DS) – required for custom development, detailing design/architecture.
  • Installation Qualification Protocol (IQP) with signed IQ Report.
  • Operational Qualification Protocol (OQP) with OQ Test Scripts and Report (showing actual vs. expected results).
  • Performance Qualification Protocol (PQP)/UAT – scenario-based tests confirming business requirements.

All evidence of testing, review sign-offs, and deviations must be archived. System Categorization and Risk-Based Validation

A cornerstone of GAMP 5 is categorizing systems by complexity and intended use, then applying a fit-for-purpose validation approach ([2]) ([1]). Table 1 above summarized the category model. The underlying logic is that simpler systems (e.g. infrastructure software, or off-the-shelf tools used without customization) carry lower risk and thus need minimal qualification, while complex configurable or custom systems warrant extensive validation.

In practice, a regulated company typically classifies each computerized system early in the project. For instance, a standard office spreadsheet without macros for non-critical calculations might be Category 1 (infrastructure/support); no formal validation needed beyond checking the correct version is installed. Conversely, a laboratory instrument’s data acquisition software may be Category 3 or 4 (COTS, possibly configurable templet depending on vendor). Enterprise systems (ERP) or custom databases would be Category 4 or 5, requiring full CSV. This classification then informs the CSV Master Plan.

Risk and Evidence. Within each category, risk-based reasoning determines how much evidence to collect. For example, a Category 4 configurable ERP module may skip some design docs (the vendor FS can stand in) but will have a detailed URS (including user roles, audit trail needs) and a risk-driven test protocol. Firms often perform a risk assessment (e.g. using ICH Q9 tools) to rank features: highly critical features get 100% test coverage, medium features may get representative sampling, and very low-risk features get spot checks.

GAMP 5 2nd Ed encourages leveraging supplier inputs where feasible. This means if a validated cloud service or commercial product is used, the company can use the vendor’s certification (e.g. ISO 13485, ISO 9001, service audits) and documented development history to reduce in-house testing ([1]). This supplier-based validation is aligned with the “increased importance of service providers” highlighted in GAMP 5 Second Edition ([1]).

Furthermore, GAMP 5 explicitly supports using automated tools to enhance risk-based validation. Test management software can perform requirement traceability, auto-generate test cases from URS, and even execute automated regression tests.   • Service/Supplier Involvement: GAMP 5 (2nd Ed) explicitly encourages regulated companies to collaborate with suppliers. The goal is to “maximize supplier involvement to leverage knowledge, experience, and documentation where possible” ([32]) ([1]). In practice, this means validating or qualifying software packages using vendor documentation (e.g. qualification protocols or audits), rather than re-testing from scratch. For instance, if using a cloud-based CRM or ERP, the vendor’s own validation evidence (GxP test reports, security scan results, change records) can be incorporated into the validation dossier. This shift acknowledges that suppliers (especially cloud/SaaS vendors) now often have certified QMS (e.g. ISO 27001 for security) and produce more validation artifacts for customers.

  • Iterative and Agile Development: The new edition recognizes that the traditional V-model is not the only methodology. GAMP 5 now “fully supports iterative and incremental methods” ([32]). An appendix added to cover agile emphasizes that standard Agile processes can align with GxP requirements. In implementation, this means firms can use iterative development cycles (sprints) but must embed validation in each sprint: for example, writing acceptance tests for each slice of functionality and performing continuous integration testing. AGILE projects in regulated environments may use an “Agile V” approach where design, build, and test happen in each cycle, but overarching documentation (e.g. a consolidated requirements baseline) is maintained. Crucially, even with Agile, the intended use and regulatory requirements must be specified up front and covered by tests. GAMP’s emphasis on critical thinking by SMEs guides when and how to apply Agile: the team should identify an appropriate level of formality (for example, using “Definition of Done” checklists to satisfy CSV criteria).

  • Automation and Tools: The second edition highlights the expanded use of tools and automation to improve control and efficiency ([1]). In practice, this can mean using automated test runners, script generation, electronic requirements management, and continuous integration/continuous validation (CI/CV) pipelines. For example, companies may adopt test automation frameworks to run regression suites against new software releases and automatically compare audit trails or data outputs. Validation documentation itself can be generated or maintained in version-controlled systems (e.g. automated tracking of change requests and validation statuses). The goal is higher quality and faster validation cycles using technology – but always under human review.

  • Data Integrity and Cybersecurity: Although GAMP 5 Second Edition did not add a separate chapter on data integrity (since ALCOA principles were already embedded), the modern context implicitly raises its profile. Implementers must ensure that the CSV process explicitly addresses ALCOA+: e.g., verifying audit trail functionality, securing electronic signatures, performing pen-testing on interfaces, and ensuring data lifecycle governance. Cybersecurity now intersects CSV: while not traditionally in GAMP 5, companies must consider system security as part of overall integrity (particularly for internet-connected/IoT systems). This includes access controls, encryption, and monitoring – which should be validated or at least assessed during CSV.

  • Quality Risk Management (QRM): GAMP 5 Second Ed reaffirms alignment with ICH Q9 risk management ([29]).
