For example, Novartis under Narasimhan has established a “US First” strategy (prioritizing American patients and regulators in decision-making) and announced a plan to invest over $23 billion in U.S. manufacturing and R&D ([35]). In 2025, Novartis joined a “most-favored nation” drug-pricing deal with the U.S. government to cap certain medication prices ([35]).

Narasimhan is also known for eagerly adopting new technologies. He frequently speaks about the impact of AI on pharma R&D and operations. In a March 2025 LinkedIn post he explained how artificial intelligence can shorten the drug discovery pipeline ([22]). He noted that finding a new medicine normally takes over a decade and ~$2 billion, with roughly 90% of early compounds failing ([36]). By using AI for tasks like predicting protein folding and scanning billions of chemical structures, “instead of taking years” to design a drug, AI can enable “significant time savings” for researchers ([22]). The State of AI in Healthcare and Pharma R&D

Pharma companies have poured hundreds of millions of dollars into AI startups and technology in recent years. It is often cited that drug discovery is extremely expensive and failure-prone: Narasimhan has noted that about 9 in 10 candidate molecules fail to reach patients ([36]). AI’s promise is to reduce these inefficiencies. Industry leaders echo this optimism. AstraZeneca’s chief data scientist said AI “helps us turn science into medicine more quickly and with a higher probability of success” ([42]). A broad industry forecast (Causeway Capital) stated that AI “will compress discovery timelines and reduce the cost of identifying new molecules” ([44]) even if it “will not eliminate” the challenges of developing a drug. The ultimate beneficiaries could be patients, through faster innovation and potentially lower costs.

However, these opportunities come with caveats. A 2025 survey by McKinsey (cited in Axios) found that only 39% of Fortune 100 boards have any formal AI oversight mechanisms ([10]). Likewise, the Trustmarque report noted that “AI adoption is outpacing governance”: while 93% of organizations use AI, only 7% have fully embedded governance frameworks ([9]). That gap is concerning in healthcare. Indeed, regulatory bodies are grappling with how to enable beneficial AI while managing risks. The FDA has already rolled out agency-wide AI pilots (e.g. a generative AI assistant “Elsa” for reviewers ([54])) and drafted guidance on AI model credibility ([55]). The U.S. Department of Health and Human Services unveiled an explicit AI strategy in late 2025, promoting innovation but also acknowledging the need for “rigorous standards” when sensitive health data is involved ([56]). The geostrategic context also complicates matters: for example, the U.S. Trump administration designated Anthropic as a “supply chain risk” and is moving to ban its use across federal agencies (including health departments) ([57]). This underscores that even as pharma embraces AI, new political and security dimensions arise.

Given this mixed landscape – great potential on one hand, and governance gaps on the other – the question of oversight is pressing. Who should help ensure AI is developed and used responsibly in healthcare? Narasimhan’s appointment to Anthropic hints that pharmaceutical leadership might have a say. To understand why this is significant, we next examine the concept of AI governance in healthcare and how pharma’s perspective can shape it.

AI Governance and Healthcare: Challenges and Frameworks

Artificial intelligence poses novel governance challenges in healthcare. Risks range from bias and safety (e.g. an AI misdiagnosing patients) to privacy and security (e.g. handling protected health data). Because healthcare decisions directly affect human lives, even small errors can be catastrophic. Moreover, the “black box” nature of many AI models clashes with medicine’s demand for explainability and accountability.

Governance here refers to the institutional and procedural mechanisms by which these risks are managed. This includes corporate oversight (C-suite and board responsibilities), internal policies (validation protocols, clinical trial monitoring), industry standards, and government regulation. In healthcare, governance also encompasses clinical ethics review boards (for trials) and patient-safety frameworks. AI systems in healthcare thus must satisfy multiple layers of oversight.

Academic and policy experts have begun articulating concrete principles for trustworthy medical AI. The World Health Organization (WHO) has issued reports (2021, 2023) on AI in health. WHO emphasizes human dignity, equity, transparency and accountability in AI for health ([58]). WHO also advocates adaptive regulation: not rigid rules, but “co- regulation” or “adaptive regulation” that can evolve with technology and build public trust ([60]). At the same time, WHO notes that current guidelines (from itself or the EU, for example) are not legally binding on member states ([61]), and it has urged legally assertive global standards.

The European Union has taken a leading role in AI legislation. Its proposed AI Act (enacted in early 2024) classifies AI applications by risk category. Crucially, most healthcare AI falls into high-risk classes under Annex III: any AI used for medical diagnosis, treatment recommendation, patient triage, or treatment selection is considered high-risk ([15]). High-risk AI systems in healthcare must meet stringent requirements (data quality, documentation, human oversight, robustness, etc.) or face penalties up to €15 million or 3% of turnover ([62]). Bouderhem et al. note that the EU framework is explicitly human-centric: it mandates that healthcare AI be based on “principles of human-centricity, trustworthiness, and sustainability” ([63]). In their view, an AI tool should even have a “Summary of Product Characteristics” and “Package Leaflet” describing its intended use and limitations ([66]). This “pharmaceutical-level” approach is meant to ensure patient safety despite AI’s rapid innovation cycles; as Perrella et al. note, AI evolves continuously, so regulations must balance innovation with robust oversight ([67]).

Beyond government rules, industry codes and boards are emerging. In corporate America, groups like EqualAI have published playbooks for board oversight, emphasizing risk management steps for directors ([68]). Surveys by the National Association of Corporate Directors indicate boards are beginning to prioritize AI governance (e.g. forming committees or appointing AI experts) ([68]). Many tech and finance boards have added AI-savvy members or advisors lately. Still, as noted earlier, many others lag: Trustmarque found only 8% of firms have integrated governance into their development lifecycles ([69]). Strengthening these structures is regarded as urgent as AI becomes business-critical. In healthcare specifically, organizations like the FDA, EMA, WHO, and patient-safety bodies are collaborating on AI guidelines. For instance, the FDA and EMA in Jan 2026 released a joint document of high-level "Good AI Practice" principles covering the entire pharmaceutical lifecycle ([11]). These principles explicitly incorporate AI into existing frameworks for drug development and manufacturing.

In summary, AI governance in healthcare is a multi-layered effort involving regulators, trade groups, companies, and now (as exemplified by this board appointment) industry leaders. On the whole, the trend is towards more, not less, oversight compared to other sectors. The pharmaceutical industry’s traditional emphasis on safety – “do no harm” – naturally extends to its approach on AI. The hiring of Narasimhan to an AI company’s board can be seen as part of this bigger picture: a way to bring that culture of rigorous review and ethical caution into the AI frontier.

Novartis CEO Joins Anthropic Board: A Governance Signal

The fact that a pharmaceutical executive now sits on the board of a major AI company is being interpreted as a deliberate “governance signal” on multiple levels. We explore these interpretations below, integrating expert viewpoints and evidence.

Domain Expertise and Cross-Industry Trust

In practical terms, Narasimhan could advise on questions like: What clinical validations would be needed before we release this AI system for use in hospitals? How do we ensure compliance with healthcare privacy laws (HIPAA or GDPR) for our data handling? What are the ethical risks if the model’s outputs influence treatment decisions? A leader who has navigated regulatory approval pathways dozens of times may be better positioned to answer such questions. For example, Anthropic noted Narasimhan’s perspective that “in healthcare, AI is accelerating solutions to some of our hardest scientific challenges” ([23]) – language that reflects biotech realities – and he himself stressed in a statement that “Anthropic is setting the standard for how AI should be developed to benefit humanity, and I’m honored to join the Board and contribute to its mission” ([23]).

Trust is also symbolic. Healthcare is a highly visible industry dealing with life-and-death issues. In theory, Narasimhan’s experience could help Anthropic anticipate regulatory or compliance issues. Unlike many tech sectors, pharmaceutical products undergo multi-phase clinical trials and detailed regulatory review; analogously, healthcare AI may in future face rigorous approval processes.

For example, the appointment comes at a time when Anthropic’s technology faces scrutiny by U.S. regulators. According to Fierce Pharma, Anthropic is embroiled in a legal dispute with the Pentagon (over a forecasting model contract) and has been labeled a “supply chain risk” by the Defense Dept. under the prior administration. Former President Trump has even issued an executive order effectively banning Anthropic’s AI (and Claude) from U.S. government use, including federal health agencies ([57]). These actions hinge on concerns about data security, sovereignty, and the potential for AI to disrupt sensitive systems. Novartis, for its part subjected to FDA oversight and U.S. trade rules, therefore has relevant experience. The public statement from Novartis on Narasimhan’s appointment carefully avoided commenting on these contentious issues, but one imagines the company’s global compliance teams will now look closely at Anthropic’s U.S. regulatory status. A link in the boardroom might facilitate dialogue with authorities if needed.

Similarly, data governance is a major issue for pharma. Healthcare data is protected under laws like HIPAA and GDPR, and patient records demand the highest confidentiality. By contrast, Anthropic’s data handling for its large language model (Claude) has raised questions. The company has claimed it builds HIPAA-ready infrastructure for healthcare use (Claude for Healthcare) ([34]), but still, questions remain about how it trains and secures AI on clinical data. A pharma leader can bring lessons from handling clinical trial data and patient information in-house. For instance, ensuring compliance with FDA’s current Good Manufacturing Practice (cGMP) and data integrity standards requires strict audit trails and validation – principles now being extended to AI tools in FDA guidance ([65]) ([11]). Narasimhan’s board role might drive Anthropic to adopt similar rigorous controls over their model development pipeline when targeting life sciences. In fact, the AI for Pharma blog notes that regulators have begun to explicitly address AI/ML under existing frameworks – such as the FDA’s 2023 discussion paper “AI in Drug Manufacturing” and a 2025 guidance on AI credibility ([65]). A pharma executive would be intimately familiar with how to fit new technology into these legacy systems, potentially smoothing adoption.

This mirrors the perspective of many AI ethicists: that technical marvels must be accompanied by robust governance. For example, Perrella et al. (2024) wrote that AI in medicine should be treated with the same rigor and oversight as a new drug undergoing clinical trials ([16]). Meeting such standards requires domain insight from pharma and medical communities. Narasimhan’s presence brings precisely that mindset into Anthropic’s leadership discussions.

On a strategic level, the move could presage tighter integration of AI into Novartis’s own future. Insider analysts speculate the collaboration could be “a prelude to further investments in AI” for Novartis ([71]). Indeed, corporate board roles often open the door to partnerships or acquisitions. Novartis might leverage closer ties with Claude’s developers to accelerate its own research pipelines. Conversely, Anthropic benefits from Novartis’s technological and clinical insights. In fact, the analysis concluded “AI is not a death sentence for big pharma; instead, it should be a catalyst for reshaping the economics of the industry, likely to the benefit of shareholders” ([72]). This suggests that AI’s role is to enhance, not disrupt, the core pharmaceutical business model.

  • Board preparedness: Surveys of corporate governance show a significant "boardroom AI gap". McKinsey research noted only 39% of Fortune 100 companies have any formal board or director-level AI oversight ([10]). Indeed, only about 7–8% of organizations have completely integrated AI governance into their development lifecycles ([73]). Bringing an “AI-naïve” pharma CEO board member does not solve this problem across industries, but it highlights it: it suggests that AI companies are acknowledging the need for outside expertise.

For example, in 2025 it launched “Elsa” – a generative AI tool built on AWS GovCloud – to help FDA scientists digest drug safety data ([54]). Experts, while optimistic, question the safeguards: for instance, Scripps researcher Eric Topol warned that using AI in FDA decision-making raises urgent questions about data security and model transparency ([74]). The political context is also in flux: the incoming U.S. government has reversed Biden-era AI guardrails in favor of more rapid adoption ([75]) ([76]), which might encourage innovation but also stoke privacy concerns. Overall, the regulatory trajectory appears to embrace AI with caution: the U.S. HHS strategy explicitly calls for “governance structure that manages risk” and resources to ensure safe usage ([77]), echoing the themes of oversight that Narasimhan advocates.

  • Case studies – promise vs. peril: We highlight two concrete examples. On the positive side, Insitro is a biotech startup that uses ML to analyze massive experimental datasets. Similarly, Formation Bio has demonstrated business value: a TIME Magazine report notes Formation’s model of buying drug candidates, running AI-optimized trials, and selling successful drugs has yielded half-billion-dollar plus deals with Sanofi and Lilly ([50]). These cases suggest that when done well, AI integration can produce concrete results and profits in pharma.

On the cautionary side, a recent Lancet Gastroenterology study (reported by TIME in 2025) revealed potential downsides of AI use. Endoscopists routinely using an AI assistance tool for colonoscopy became “over-reliant,” such that when the AI was removed their performance dropped sharply ([20]) ([21]). The adenoma detection rate without AI fell from 28% to 22%. Researchers attributed this to doctors’ reduced vigilance when aided by AI. This empirical finding underscores a governance concern: AI systems can inadvertently deskill human professionals if oversight and training are insufficient. It highlights why experts stress that AI tools must be carefully managed and explained to clinicians – an insight that a rule-driven industry veteran like Narasimhan would appreciate.

FiercePharma notes that Narasimhan appears to be the first pharma executive on the board of a major AI company like Anthropic ([25]). His predecessor Joe Jimenez (also an ex-Novartis CEO) joined a smaller AI advisory board earlier, but Narasimhan’s case is on a much grander scale ([25]). Commentary around the announcement emphasized the message: having a pharma CEO on the board means the healthcare use-case is taken seriously. Case Studies: AI in Action in Pharmaceuticals

To ground the discussion, we present selected case studies illustrating how AI is being used in pharma and healthcare, and what issues have emerged.

Accelerating Discovery (AlphaFold and Beyond)

Example: AlphaFold and Novartis. One of the landmark advances in AI-driven biology was Alphafold, Google DeepMind’s AI system for predicting protein structure. In 2021, Alphafold’s accuracy turned heads (even winning a protein-folding “competition”), and pharma quickly took notice. Novartis partnered with DeepMind’s spin-off Isomorphic Labs in 2023 to leverage next-gen protein prediction in small-molecule drug discovery ([4]). This collaboration (worth up to $1.2 billion) aims to identify new molecular structures that bind disease-related protein targets. The idea is that, by knowing a protein’s 3D shape, chemists can in silico screen trillions of compounds instead of relying on slower lab searches. Preliminary results have been promising: press reports indicate that within the first year, Isomorphic’s newer models (beyond Alphafold) have already been used to generate viable drug candidates in partnership with Lilly and Novartis ([4]).

So far, Novartis has been transparent about its data sharing and partnerships (see businesswire announcements of the deals).

Streamlining Clinical Trials (Formation Bio)

Example: AI for Trial Operations. Formation Bio is a venture-backed biotech that uses AI primarily for trial management rather than discovery. Instead of hoping to invent a novel molecule, it acquires existing promising drug candidates and offers a one-stop shop: using AI to recruit patients, design trial protocols, and expedite the approval process. According to TIME (Feb 2026), Formation uses “AI to accelerate administrative tasks such as patient recruitment, regulatory filings, and matching drugs to specific diseases” ([49]). The company’s model is: acquire 3–4 promising compounds per year, run the trials in-house, and then out-license or sell successful treatments to larger pharma.

Data Point: Formation CEO Ben Liu reports that since inception, they have successfully completed two drug trials: one sold to Sanofi for about €545 million, and one minority stake deal with Eli Lilly worth nearly $2 billion ([50]). These high multiples demonstrate that even at this small scale, “AI-assisted trials” can create substantial value. The key efficiency comes from reducing human labor: Liu said, “If you can run trials cheaper and faster, and instead of 100,000 people, you employ 100 people using these AI systems to do most of the knowledge work, you should be able to offer drugs with far more expanded access at lower cost” ([51]).

Governance Note: Formation Bio’s approach represents innovation in business model. However, it raises new oversight questions. If an AI-designed protocol expedites a trial, regulators must ensure it still meets safety and statistical standards. Formation argues that AI can handle much of the grunt work, but there are concerns that over-zealous automation could overlook nuanced ethical considerations (consent, patient monitoring workflows, etc.). In this case, Formation has shown willingness to exit (sell the drugs) once unsure, reducing long-term risk. Its success has encouraged other entrepreneurs to attack trial inefficiencies with AI. It highlights how AI governance can extend beyond R&D into quality control of clinical operations.

Insitro: Merging Data and Biology

Example: Machine Learning for Disease Mechanisms. Insitro, a San Francisco biotech co-founded by Stanford computer scientist Daphne Koller, exemplifies applying deep learning to upstream biological data. The idea is that massive genomic and imaging datasets can be mined by AI to uncover hidden disease pathways. In a 2024 AP interview, Koller explained that Insitro “unravel [s] the underlying complexity of heterogeneous diseases” by finding the right “therapeutic hypothesis” for specific patient sub-populations ([78]). The company collected big blood sample and tissue datasets and built ML models to associate molecular signatures with drug response.

Business Model: Insitro has attracted partnerships with major pharma: by 2024 it had deals with Eli Lilly and Bristol-Myers Squibb to co-develop treatments in metabolic disease, neurology, and degenerative disorders ([18]). These deals demonstrate pharma’s appetite for AI-driven target discovery. In theory, AI could identify a promising protein to drug against that had been missed by traditional methods.

Governance Note: Projects like Insitro’s raise classic drug development questions in a new setting. When an AI model flags a new target, it still needs to be validated in the lab and clinic. The potential pitfall is trusting an AI “hypothesis” that might be spurious or biased (garbage in, garbage out). Insitro’s co-founder emphasizes understanding the ML models’ limitations: she notes that successes have come only when the biological system is sufficiently well-characterized ([82]). This aligns with the broader theme: humans must interpret and test AI findings rigorously.

Human Factors: The Cautionary Edge

Example: Skill Degradation when Using AI. Not all outcomes of AI integration are positive. A very recent study highlights a potential unintended consequence of AI assistance. In Poland, a multi-center clinical trial introduced an AI system to help endoscopists detect precancerous polyps during colonoscopies. Doctors who used the AI agent for several months subsequently performed worse when their AI aid was removed ([20]) ([21]). The Lancet Gastroenterology paper reported that after three months of AI use, unassisted doctors’ adenoma detection rate fell by about 20% (from ~28% to ~22% compared to pre-AI baseline) ([21]). Researchers concluded the clinicians became over-reliant on the AI (and less “motivated, less focused, and less responsible”) ([20]).

This case study is relevant to governance: it shows that how AI is deployed matters. If an AI tool is simply dumped into clinical workflows without training, oversight, or fallback procedures, there can be harms. It underscores Narasimhan’s point: “speed isn’t the goal” alone ([14]). Doctors need to understand AI’s strengths and limitations. Governance frameworks (whether corporate or government) must ensure that AI integration comes with clinician education and performance monitoring. This study has prompted calls for cautious AI implementation guidelines – exactly the kind of insight that a medically trained board member would appreciate.   • Cross-Industry Boards May Multiply. Other sectors will watch closely. If the Novartis appointment is viewed positively by patients, regulators, and markets, other AI companies may seek out health-sector board members, and vice versa. Pharmaceutical groups might similarly invite AI experts onto their own boards (some already have: the Novartis CEO’s predecessor, Joe Jimenez, joined an advisory board of AI company Aily Labs ([25])). Biotech startups often include tech-industry academics or entrepreneurs on scientific advisory boards; making them part of full corporate governance boards is a new step. As one vessel’s Axios report put it: “Company boards scramble to adjust to AI,” with organizations publishing playbooks for directors ([68]). We may soon see “AI committees” composed of multi-disciplinary directors, including medical professionals, on more corporate boards.

  • Strategic Partnerships and Investments. Novartis may leverage Narasimhan’s role to forge deeper ties with Anthropic. While no formal joint venture has been announced, the deal could presage shared R&D projects or licensing agreements. Born out of such relationships, we could even imagine future drug discovery workflows where an Anthropic-derived model co-designs molecules with Novartis chemists. Conversely, other pharma majors will likely strengthen AI divisions. The industry already sees a race: AstraZeneca lists AI funds among its recent top holdings ([83]), GSK earmarked $1.2 billion to implement AI in its U.S. manufacturing ([43]), and Johnson & Johnson, Pfizer, and others have AI initiatives underway (though not detailed here). Narasimhan’s move blurs corporate lines; Novartis has effectively inserted itself into Silicon Valley’s AI ecosystem. Investors should watch if health-sector M&A of AI startups accelerates.

  • Technology and Product Development. Anthropic will likely continue expanding its healthcare-specific products. They have already introduced HIPAA-compliant “Claude for Life Sciences” and “Claude for Healthcare” models to assist researchers and clinicians ([34]). With Narasimhan aboard, the company may push further into specialized healthcare applications – for example, speeding up regulatory filing summaries, modeling disease pathways, or synthesizing medical knowledge. However, competition among AI providers in health is heating up. OpenAI, Google, Microsoft, and others will seek to embed safeguards in their systems (some government agencies have already talked to multiple AI firms, e.g. cderGPT discussions ([84])). The presence of a pharma CEO at Anthropic may nudge these companies to consider partnering with drug companies, not just insurers or providers.

  • Regulatory Influence. In future AI regulations, we can expect the healthcare industry to fight for favorable frameworks. For instance, pharmaceutical lobbyists and executives may push to ensure clinical AI is included in medical-device or drug-reg regimes (like the AI Act’s high-risk category). Conversely, AI companies will cite Narasimhan’s involvement to demonstrate they take “human health” seriously, possibly softening legislative zeal for bans. There is already tension evident: while regulators like the FDA welcome AI to improve efficiency ([85]) ([54]), lawmakers are debating privacy and even national security aspects (e.g. HHS reports reveal some political opposition to big tech handling health data ([86])). Narasimhan and Novartis executives, as known corporate citizens, may be key voices advising governments on where to draw lines (e.g. data-sharing rules, clinical trial integrity, patient consent).

  • Governance Evolution. Perhaps most importantly, this event may accelerate the co-evolution of AI development and governance.
