The OncoLife™ Survivorship Care Plan program is a free, publicly accessible, internet-based tool developed by a dedicated team of oncology nurses and physicians at OncoLink, a web-based resource of the Abramson Cancer Center of the University of Pennsylvania. Launched in 2007, the program addresses the need for comprehensive guidance on the long-term effects of cancer treatments for adult cancer survivors. The tool is based on the National Institute of Medicine’s recommendations for survivorship care. Users, including patients, caregivers, or healthcare providers, complete a brief online questionnaire that includes demographics and disease-specific questions regarding cancer type, surgeries, chemotherapy/biologic therapies, and radiation therapy. OncoLife then produces a personalized, comprehensive long-term survivorship care plan. The resulting plan includes a treatment summary, information on potential late- and long-term effects, recommendations for follow-up care, cancer screening, and a healthy lifestyle. The program offers two versions: a patient version and a healthcare professional version, the latter of which allows clinicians to remove side effects they deem irrelevant for a specific patient. The plan also provides a "Healthcare Provider Summary" for easy reference by primary care physicians and other specialists. The tool is available in both English and Spanish and has generated over 60,000 care plans.
