Journey Forward is a collaborative initiative that developed the Survivorship Care Plan Builder (SCPB) software and related tools to address the critical need for coordinated follow-up care for cancer survivors, as identified by the 2005 Institute of Medicine (IOM) "Lost in Transition" report. The software is a centerpiece resource, provided free of charge to individuals and institutions by the collaborating organizations, which include the National Coalition for Cancer Survivorship (NCCS), UCLA's Cancer Survivorship Center, the Oncology Nursing Society (ONS), Anthem, Inc., and Genentech.
The SCPB software, and its later web-based version (SCPB 2.0), allows health care providers to quickly and easily generate customized treatment summaries and follow-up care plans. The plans are based on evidence-based guidelines, such as the American Society of Clinical Oncology (ASCO) templates and surveillance guidelines for specific cancers (e.g., breast, colon, lung, lymphoma, and generic). The tool captures essential post-active treatment information, presents it in a manner easily assimilated by patients and their Primary Care Physicians (PCPs), and assists oncologists and PCPs in delivering follow-up care.
The software also supports data integration from cancer registry systems (like ERS and C/NET Solutions) to efficiently and accurately import diagnosis and treatment summary information. The resulting care plans can be exported to formats like Microsoft Word, Microsoft Excel, and PDF. The program aims to enhance communication and coordination, improve post-treatment care, and empower patients to be more involved in their survivorship experiences.
Additionally, Journey Forward offers a patient-facing mobile application, My Care Plan, available on the Apple App Store and GooglePlay, which allows survivors to initiate their own care plan to be finalized with their healthcare team.
