Axxess HIS Tracking, also known as the HIS Export Manager, is a critical component of the Axxess Hospice electronic medical record (EMR) solution designed to ensure compliance with the Hospice Quality Reporting Program (HQRP) mandated by the Centers for Medicare and Medicaid Services (CMS). The module is specifically built to manage the Hospice Item Set (HIS) admission and discharge documents.
Key Features and Capabilities
The software automatically collects data from clinical documentation, eliminating manual data entry and reducing errors. It features a centralized HIS Export Manager with three main tabs: Pending Approval, Export Ready, and Export History. This structure provides an organized workflow for hospice agencies.
- Automated Data Collection: Data for the HIS is automatically populated from completed clinical documents.
- Color-Coded Deadline Alerts: The system provides color-coded alerts and a 'Days Remaining' countdown to track critical HIS deadlines (e.g., 14 days for Admission approval, 7 days for Discharge approval, and 30 days for submission to the QIES ASAP system).
- Review and Editing: Users can review, edit, and approve HIS Admission and Discharge documents, with the system recording which user completed each question for accountability.
- File Generation: It generates HIS upload files in the mandated XML file format, which are then placed into a ZIP file for acceptance by the QIES ASAP system.
- Correction and Cancellation: The system allows users to easily generate correction or cancellation files for previously submitted HIS documents.
- Bulk Management: Users can perform bulk approval, bulk file generation, and bulk status updates for multiple HIS documents.
- Service Utilization: For discharge due to death, the HIS Discharge document automatically generates Service Utilization data for Section O.
Target Users and Use Cases
The primary users are hospice administrators, quality assurance staff, and clinical reviewers within Hospice and Palliative Care organizations. The main use case is ensuring timely and accurate submission of the Hospice Item Set to maintain compliance with HQRP requirements and avoid payment penalties.
